ViridianCares supports your practice and your patients by providing access and reimbursement support, including helping with enrollment, insurance processes, affordability programs, and treatment coordination to help your patients start and stay on treatment.
Enrollment Steps
Enrolling your patient is a critical first step. There are 2 ways to complete the enrollment form:
1. Complete form, ensure patient reviews and signs, and then fax all 4 pages to 1-833-726-5151.

2. Complete the EZ Enroll enrollment form electronically
ViridianCares Co-pay Program
Eligible Lumvoa patients may pay as little as $0 in out-of-pocket costs*
The ViridianCares Co-Pay Program offers co-pay assistance for eligible patients. Commercially insured patients may pay as little as $0* for medication and infusion-related expenses. Terms and conditions apply.
For financial assistance programs, the process begins when your office enrolls a patient by submitting the appropriate enrollment form. Once an enrollment form is submitted, ViridianCares will work with your patient to further determine eligibility and benefits.

Patient Access Liaisons
Once a patient is enrolled, a dedicated PAL serves as a single point of contact for your practice, your patient, and infusion centers.
Your PAL is available to provide support for your patients. PALs can help navigate a wide array of issues, such as patient onboarding, insurance approvals, prior authorizations, and treatment logistics—all with the aim of helping you start and keep patients on therapy.
